21CCLC Parent Survey

21CCLC Parent Survey

Dear Parent/Guardian,

Thank you for taking the time to provide us with your important feedback about your child’s participation in our program.

The Weikart Center is a non-profit agency that helps afterschool programs learn how to improve. They will be collecting responses on this survey in order to help us understand how we can best serve students. The responses to this survey are confidential and anonymous so your responses cannot be linked back to you.  We are asking parents and caregivers to tell us about their experiences and their child's experiences in their afterschool programs. Your input is very valuable and will help in making decisions about how to improve our afterschool program. Please answer the questions as honestly as you can. You do not have to answer any of the questions if you don’t wish to, and you can stop doing this survey at any time.

This is an online survey. If you do not have access to the internet, we will provide access to the survey at the school. Please let us know if you want to take the survey when you pick up your child.

Below are instructions:

1.     To get to the survey, please click on this link:

2.     On this webpage, you should see a section called “2018-19 Online Evaluation Surveys.” In this section you will see five different surveys listed. Please click on Parent/Caregiver survey.

3.     Please note that there are two versions of the Parent/Caregiver survey, English and Spanish.  Please pick the one that is most appropriate for you.

4.    You will have until April 26thto complete the survey.

Please make sure you read each question carefully and select the response that best fits you. If you want to change any of your answers, just click on the new answer.  Thank you for your time.  We truly value your input.

about the 21st Century community learning centers Program

The 21st Century Community Learning Centers (CCLC) program is authorized under Title IV, Part B of the Elementary and Secondary Education Act, as amended by the No Child Left Behind Act of 2001. The 21CCLC program provides federal funding for the establishment of community learning centers that provide academic, artistic and cultural enrichment opportunities for children, particularly students who attend high-poverty and low-performing schools, in order to meet state and local standards in core academic subjects such as reading, math and science. This program is also intended to offer students a broad array of activities and to include families and the community in the educational process.

The program will be Monday through Thursday. There will be no program on the Fridays we are at school. The program will begin right after school. The students will go to the cafeteria for snack, have a short recess followed by homework help and then enrichment. (STEM, Literacy, Tutoring, Entrepreneurship, Art, Music, Physical Activity, Community/Service Learning, and Mentoring)
We will also offer classes on drug prevention, violence prevention, youth leadership opportunities, and college and career readiness.
The high school will have math tutoring, drama, art, and more!
We are servicing PK-12th grades, plus we will provide mulitiple Adult Education/Family Engagement opportunities such as Financial Planning, Love & Logic parenting classes and family reading nights throughout the year.
Students must be picked up at or before 5:45 pm in the circle drive.
If you have further questions, please call Danyele Walker at (405) 969-3738 ext 4150
