Teacher Evaluation |
| Portfolio Presentation 60 points |
The student name, science class, and semester are clearly marked on the outside of the portfolio cover.
(A "magic marker" is used.) |
5 | ||
| The first page of the portfolio is a title page created with Microsoft Word. The title page includes the student's name, science class, and the semester represented by the portfolio.
(The construction of the page demonstrates the student's skill using the "extras" in Word, such as WordArt or clipart.) |
10 | |||
| The portfolio is organized from front to back, with the most recent pages at the back.
(Daily pages and assignments are found together in chronological order.) |
10 | |||
| All assignments, practice problems, and "test your concept understanding" questions for the grading period have been completed and are found in the portfolio. | 10 | |||
| All entries in the portfolio are either neatly written in ink or typed and printed.
(Pencil is not used in the portfolio.) |
10 | |||
| The neatness of the portfolio indicates that the student takes pride in their work and desires to be successful in class.
(Examples: all entries are carefully written using the same color pen, no pages are torn or dirty, no "spiral notebook" pages are used, and no pages have drawings or marks unrelated to the assignment.) |
10 | |||
| When the portfolio is presented for grading, all extra blank pages have been removed and there are no loose pages in the pockets of the portfolio. | 5 | |||
| Daily Comments 30 points
|
Each day of class is clearly identified with a heading: such as "Monday - March 25, 2004". | 10 | ||
| There is a diary-like entry every day, giving the student's reactions and impressions about things that happened in class that day.
(For days you are absent, the reason for the absence will fulfill this criteria.) |
20 | |||
| Student Improvement 10 points |
There is obvious improvement in the quality of the student's work during the semester represented by the portfolio.
(Indications of improvement include: more detailed comments about class impressions and answers to assignment questions are more complete.) |
